The arm bike isn’t magic. Pulleys aren’t rehabilitation. And pretending a 1-lb increase is meaningful loading is an insult to human physiology. This is what happens when PT becomes a factory. Templates replace thinking. Timers replace coaching. And “busy” gets confused with “better.” I worked in that world. I played the game. I know exactly why this happens—and it has nothing to do with what’s best for the patient. Rehab should look different week to week because YOU are different week to week.
